Disease Management

The CCHN works with community clinics and health centers to design disease specific key measures, and subsequent compliance with these measures, based on nationally recognized standards of care with the ultimate goal of improving patient and population health outcomes.  Areas of focus include asthma, diabetes and cardiovascular disease.
