CCHN Services

The Community Clinics Health Network (CCHN) is a non-profit 501(c)(3) subsidiary of the CCC that was founded in 1993 to provide managed care support to San Diego's community health centers.

CCHN continues to develop and implement collaborative and innovative programs and services to support community clinics and health centers and the patients and communities they serve.

Current Initiatives

Specialty Care: Collaborate with partners across the continuum of care to ensure timely and appropriate access to specialty services, procedures and outpatient surgeries for clinics patients.

Information Technology: Through the TSO, support clinic teams to select and implement electronic medical record systems, patient registries, pharmacy management systems and other clinical information systems to increase efficiencies for clinic operations and to improve quality of care and health outcomes for clinic patients.

Care Coordination: Connect clinics and hospitals to establish medical homes and to increase access to care and information exchange across the continuum of care to improve care coordination and health outcomes for clinic patients.

Accomplishments

  1. In 2009, CCC/CCHN secured $2.9 million in funding for clinics and specialty providers to cover under insured or uninsured patients.
  2. CCHN serves as the Countywide Coordinator for the Oral Health Initiative, a project in which clinics have provided treatment to almost 15,000 pregnant women and children from birth through age five. Intensive care coordination was also provided to 4,800 families.
  3. Earned reimbursement from health plans using Pay-for-Performance.
  4. Worked with County and Human Services Agency on the Health Coverage Initiative.
  5. Exceeded national benchmarks for diabetes care.
  6. Provided video conferencing support services to clinics.
  7. Improved information sharing between health centers and UCSD through the Community Access Program.
