Aspirin, Lisinopril, Lovastatin (ALL) Project

CCHN continues to work closely with Kaiser Permanente (KP) to implement the Aspirin, Lisinopril, Lovastatin (ALL) Intervention in community health centers in San Diego and Imperial Counties with the goal of improving health outcomes of patients with diabetes at risk for cardiovascular disease (CVD).

In coordination with KP, CCHN is working closely with the Physician Council to develop and implement project activities, including: collecting, summarizing, and presenting data and information on ALL intervention to clinic providers and staff; forming a Physician Council subcommittee on ALL intervention project; and developing criteria, materials, policies and procedures, protocols, forms and data collection and reporting tools for ALL intervention.

The project will identify patients with diabetes at high risk for CVD in community clinics and health centers, and apply the ALL intervention as an evidence-based, cost effective means of improving health outcomes. The goal of the project is to improve health outcomes of community health center patients 50 years and older with diabetes at risk for cardiovascular disease (CVD).  CCHN and participating community health centers will work with Kaiser Permanente (KP) in phases to develop and implement this program. Phase 1 focused on formative activities, developing the program with KP and the Physician Council, educating clinic physicians and staff of the ALL intervention, and gathering baseline data. In Phase 2, CCHN implemented and evaluated the ALL intervention at 3 clinic sites.  There are currently five clinics participating in the project.

The ALL program was developed in the KP Health system to target members at high risk for cardiovascular disease (CVD).  This program has been modeled by a robust program named ARCHIMEDES to provide an evidence based, cost effective method of CVD prevention in these high-risk individuals. By providing this program KP has significantly increased use of preventative medications for CVD, created a replicable model, and has demonstrated leadership in CVD population care prevention.
