The mission of the San Diego Learning Communities is to prepare and support the leaders of San Diego's behavioral health and   primary care systems of care in becoming effective and passionate change agents in an evolving integrated health care system.

The goal of each Learning Community is to support integration change leaders in San Diego by incorporating: (1) emerging integration best practices and resources; (2) organizational change leadership and management curriculum; and (3) trust-building approaches for partnering providers.

The work of the San Diego Learning Communities will be guided by the "Vision of Primary Care and Behavioral Health Integration" as defined by the County of San Diego: Health and Human Services Agency.

LEARNING COMMUNITIES (LC) are designed to help you and your organization successfully implement service Integration. The San Diego Learning Communities are aligned with the County of San Diego Health and Human Services Agency's Behavioral and Physical Health Integration Vision, "Shared Population Management".  This vision provides comprehensive, coordinated care that emphasizes continuous healing relationships instead of reactive, episodic treatment.

HOW TO APPLY:

Any interested agency/clinic/organization will be asked to complete a brief application.

WHAT'S EXPECTED OF PARTICIPANTS?

Participating agencies/clinics/organizations will designate at least 2-3 senior staff and providers to participate in the LC meetings and webcasts. Participants are required to make a 9 month commitment to their LC Cohort. This commitment includes three in-person, one day meetings and six one-hour webcasts/coaching sessions. Participants will also complete pre and post Integration needs assessments, sign an MOU, and complete/execute an individualized change plan around Integration and Change Management.

WHO SHOULD PARTICIPATE?

Senior leaders and providers working in the San Diego County region from community clinics and health care organizations, community mental health agencies, and alcohol and other drug service providers.

FACILITATORS:

The Community Clinic Health Network will facilitate/coordinate the Learning Communities, in cooperation with the County of San Diego HHSA Behavioral Health Services; and Change Leadership and Integration Experts Personal Mastery Programs and National Council for Community Behavioral Healthcare.

LOCATION:

All in-person meetings will be held at the Council of Community Clinics in Mission Valey (7535 Metropolitan Drive, San Diego, CA 92108). Other sessions will be via webcasts.

SCHEDULES: Three Learning Community Cohorts are scheduled between January 2012 and May 2013

  • LC-A (January- September 2012);
  • LC-B (May 2012 - January 2013);
  • LC-C (September 2012 - May 2013)
