San Diego Integration Institute (I2)
In January of 2007, the County of San Diego Health and Human Services Agency, Behavioral Health Services Division funded the Community Clinics Health Network (CCHN) via contract #532853 to implement the Mental Health and Primary Care Integration Services project.
Effective May 1, 2011, contract #532853 was amended to include funding to develop and implement an Integration Institute. The Integration Institute began on May 1, 2011.
Goals / Purpose
The goal of the Integration Institute (I²) is to support Behavioral Health (BH) providers, and Primary Care (PC) providers to make progress in enhancing intra-organization integration and in achieving inter-organization, bi-directional integration of primary care, behavioral health, and ADS services.
