HISTORY

Project ALL was developed in the Kaiser Permanente (KP) health system to target members at high risk for cardiovascular disease (CVD). The program was designed to provide an evidence based, cost effective method of CVD prevention in these high risk individuals. By implementing ALL, KP significantly increased the use of preventative medications for CVD, created a replicable model and demonstrated leadership in CVD population care prevention.

In 2008, KP provided funding for a pilot project for the Community Clinics Health Network (CCHN) to bring Project ALL to San Diego community clinics. The project was successfully implemented in 16 clinic sites. The community clinics and KP have a long history of working together in partnership to improve the health of patients and communities. Together, the two groups share a common mission and a similar diverse patient population.

By taking a proven clinical protocol and successfully implementing it in a community clinic setting, KP and Project ALL created a win for everyone involved. Clinic providers learned from KP's success (and vice versa) and clinic patients experienced improved quality of care and health outcomes.

CCHN continues to hear from our providers about how much Project ALL benefitted their patients and from clinic patients about how much Project ALL benefitted their lives. CCHN is looking forward to working with Kaiser and clinics throughout Southern California to expand the program to additional patients.
