CCHN Projects

The CCHN currently has a number of grant-funded projects underway with its participating clinics.  These initiatives leverage the collaborative efforts of the Physician Council with our community partners to develop best practices for treating medical conditions that are prevalent in community clinic patients. 

The Physician Council, comprised of clinic medical directors, has currently identified pediatric asthma and adult diabetes as areas of focus.  Additionally, CCHN staff is collecting Body Mass Index (BMI) information while performing chart audits.  Through this process the CCHN will have baseline information regarding obesity in pediatric patients seen at the clinics.
